Stem 30-60cm in height with up to six ovate to lanceolate leaves. Flowers usually in pairs but can be one to three, the lip obliquely ovoid, white, 2-3cm long, sepals and petals brownish-purple, the petals spirally twisted. Mid-California to Wyoming and north to Canada in moist, mixed evergreen woods.
